Project Output and Monitoring Questionnaire - Diwyllesiant Support Fund

Congratulations on completing your project funded (in full or partly) through the UK Shared Prosperity programme.

In accordance with the conditions of the grant we would be very grateful if you could take the time to complete this short survey, it should only take a few minutes. The information will be used to report on the outputs and results of the programme. It will assist Cyngor Gwynedd with the information to ensure future provision of such grant funding programmes. 

If you do not complete the questionnaire and provide information on the e of your grant, Gwynedd Council reserves the right to implement a process to reclaim part / all funding.
